Service Description
Luxe Lips Training – Registration Details If you’re ready to enroll in our Luxe Lips Micropigmentation Training, please carefully review the details below before submitting your request. How to Request Your Training Date: • Luxe Lips Training is scheduled by request only. • You must request two consecutive days for your training, choose any day available in the booking calendar, upon request, we will contact you to discuss. Class is 2 days 15 hours long. • Once we receive your request, our team will follow up to confirm instructor availability. Class Schedule • Arrival Time: 8:45 AM • Training Hours: 9:00 AM – 5:00 PM • Instructor: Sandra McGuire Payment Details • Deposit Required: $300 (non-refundable) • Deposit is credited toward your total tuition • Can be paid via cash or credit card • Balance is due in full on the first day of training • A training kit is included and will be provided in class Deposit Policy • The $300 deposit is non-refundable • Students may reschedule and transfer the deposit once, to a new available date • To transfer your deposit, you must give at least 7 days’ notice before your original training date • No-shows or late cancellations will result in forfeiture of deposit Model Requirement • Students are required to provide a model for their hands-on portion of the training • For beginner students, models must attend on Day 2 of the course • Full model guidelines and schedule will be provided in your confirmation email By Registering, You Agree To: • All terms and conditions outlined above • Providing a model as required • Attending both consecutive training days in full • Being responsible for final payment on the day of class Let me know if you’d like a version for a form or online booking system!
Cancellation/ Reschedule Policy
MICROBLADING TRAINING – CLASS INFORMATION Instructor: Anny Availability: First come, first serve DEPOSIT & PAYMENT POLICY Deposit: Non-refundable (see policy below) Total Cost: $2,700 Balance After Deposit: $2,200 (cash, zelle) $2,250 (credit card) – includes a $50 transaction fee Credit Card Payments: You must be the cardholder. If you are not the cardholder, request a Credit Card Authorization Form to be filled out. If paying the balance with a credit card, you must bring the authorization form and a copy of the cardholder’s ID on the day of class. REGISTRATION REQUIREMENTS ✅ To complete your registration, you must: Sign and return the attached document (sent with this invoice). You may sign on your phone or print, sign, and email a scanned copy or photo of the signed form. Submit payment. ⚠️ IMPORTANT: Registration is only complete once we receive your signed form and deposit. Confirmation Email: You will receive a confirmation email within 24 hours after payment and form submission. Please check your inbox for details. TRAINING DETAILS Students must bring their own model on Day 2. Model’s arrival time will be provided in the confirmation email. No cost for the model to receive microblading from the student during class. TRAINING KIT A training kit will be provided in class. CHANGE DATE POLICY Students may reschedule one time only, with at least 15 days’ notice before the scheduled class. If less than 15 days' notice is given, the deposit is non-transferable to another class date. If Anny-M LLC needs to cancel the class for any reason, the deposit will be transferred to the next available class assigned by Anny-M LLC. LICENSING INFORMATION Students are responsible for any licensing requirements by their city/county's Department of Health, including state fees. In Florida, you must obtain a permanent tattoo artist license after training, once you have a fixed establishment to work. Licensing will be discussed in class.
